Immigration Physical requires:

1.  Vaccines

If your vaccination report is incomplete, we will guide, and advise you on how to complete it, including blood work for proof of immunity

What to expect from your immigration physical:

• Medical History and Physical Examination  - A comprehensive head to toe exam by our medical provider. In order to fill out your form I -693 - It is important to call our office an send us your vaccination records for physician review, our physician will inform you of which shot are missing.  No vaccines or blood work are done on site, therefore vaccines can be obtained at Walmart or Walgreens, no prescription is needed to get vaccines.  If you have health insurance, the pharmacy can directly bill your insurance for the cost of the vaccines.

       For blood work, we recommend emailing your lab results before your appointment to be reviewed by the provider.

In order to get your Form 693 completed, all required immunizations and Lab results must be obtained and reviewed by our provider.​

The visit therefore starts with a review of your vaccination and lab results and ends with a face-to-face exam with our provider.

A full payment is expected prior to the face-to-face visit.
